GENERAL INFORMATION

Luminate is open Monday – Friday from 9:30am to 5pm

Please note that our office will be closed in observance of the following holidays: President’s Day, Good Friday, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas, New Years and the week between Christmas and New Year’s to spend extra time with family and friends.

We follow the Tulsa Public School recommendations for closing to keep our guests and staff safe.

Our office is conveniently located off the Broken Arrow Expressway on the North East side of Utica and Cherry Street in Tulsa, OK. We are in a historic 100 year old Tulsa Italian Villa style office, you will find 2 parking spaces at the front door and 10 spaces behind the building. Because of the historic nature of the building, original stairs exist and we are not handicapped accessible. 

  • Completed new patient forms (this can be done online as forms will be emailed, or in clinic)
  • List of current products, medications and medical conditions
  • Valid ID

Please call our office at least 24 hours in advance of your appointment to avoid any cancellation fees. We’ll work with you to help you find another date that fits your schedule.

All appointments require a credit card number to be kept on file. Please notify our office at least 24 hours in advance when cancelling or rescheduling appointments. Failure to contact us within 24 hours of your scheduled appointment will be considered a “Late Cancellation/No Show” and the following fee structure will be applied to the credit card on file.

  • If there is a Late Cancellation/No Show, the card on file will be charge $50 PER EACH half-hour booked. This policy helps us ensure our availability to give you and our other patients room to book with us. Late Cancellation/No Show fees will not be applied on top of non-refundable deposits.

If you are more than 15 minutes late for your appointment, we reserve the right to reschedule your appointment or shorten the duration of your service. This may result in a different charge for services rendered OR a fee based on our “Late Cancellation/No Show” policy.

Absolutely yes! We have many patients that do this, especially those that live far away or are in a time crunch as it can help you save time by reducing the number or visits. But we must make sure the treatments are done in the correct order for optimal results, so we would recommend you call 918-340-7090 and have our Guest Relations help you schedule this.

We will conduct a thorough examination on your first visit to determine the best course of action to achieve maximum results. After completing your intake form, your scheduled provider will guide you to one of our treatment rooms to begin your “Get to Know You Session.” During this consultation, our staff will ask you about the goals you wish to achieve with our services and will create a personalized treatment plan for you. This plan will include a list of medical-grade products and services, as well as their pricing, and treatment instructions. We will be able to reference back to this guide throughout the duration of your treatment.

Many health conditions are indicated on the skin and can affect cosmetic health, so your medical information is very important in diagnosing issues or concerns with your skin and body.

In consideration of all our guests, we ask that you silence cell phones and pagers when visiting our facility.

Please arrive 5-10 minutes prior to your appointment. This allows time to check in, change or preparation for treatment and begin your treatment.

We suggest that you wear comfortable clothing. Special attention and careful draping for privacy are observed during all body treatments.

We love children too, but are unable to accommodate young children during your visit to our clinic with services that involve blood, needles, chemicals or laser. Children are welcome if they have an appointment for services and are chaperoned by a parent or guardian.

One of our highest commitments is your privacy. Your personal information is confidential. This includes your credit card info, address, phone numbers and email address on file. We do not share your personal information or email with third parties. We value and respect our guests’ privacy and preferred methods of receiving (or not receiving) invitations & communications from us.

FINANCIAL

We accept all major credit cards, debit cards, and cash, as well as Cherry financing available. Payment is due upon conclusion of all appointments with services rendered unless approved otherwise.

- We believe that all people should have access to their desired treatments. Because of this, we’re happy to announce our partnership with Cherry Payment Plans.

Yes and no, for your first visit, we charge $50 and recommend beginning with a “Get to Know You Session” to create a customized treatment plan for you. We believe that this is a vital step in maximizing patient results, so the $50 fee for this consultation will be applied as a credit on your account towards your next appointment.

We require a $250 non-refundable deposit for all procedures over $499. We may require deposits on some services or promotional pricing, especially during busy occasions, i.e. holidays, weekends, etc…

 

Luminate Clinic does not accept CareCredit Payments at this time, However, we are pleased to offer customizable payment plans through our partnership with Cherry.

Insurance companies dictate to medical providers what is "necessary" and what is "elective" without even knowing who we are. They ignore the principles, goals and ethical views of providers and try to limit our options available to our patients based on what they will reimburse for. It is crucial that I provide the best possible recommendations based on the science, research and effectiveness of the services, products and procedures that are proven to deliver results.

I have personally negotiated the best possible pricing for all labs we offer and will continue to make sure our pricing is as affordable as possible.

Give the ultimate gift of wellness. This unique gift of bliss will be appreciated by everyone. Gift cards are available in any denomination and can be purchased by telephone or in our clinic. Gift Cards expire 12 months from date of purchase.  Gift Cards are not redeemable if lost or stolen. Gift Cards are non-transferable and expiration dates may not be changed or extended. If an appointment made to redeem your gift card is not cancelled at least 24 hours in advance your gift card will be forfeit.

PURCHASE ONLINE

All payments are final and no refunds will be processed. In-person follow-ups are required for questions and concerns, they will not be handled via text, over the phone or on social media.

We offer a 100% satisfaction guarantee on all of our products. If you decide that it is not the right product for you, you may return the unused and unopened product within 7 days of original purchase to receive a credit for the full amount on your Luminate account. No cash or credit card refunds – exchanges…. credit only.

If you purchased a package a series to receive a discount,  you will be charged full price for the previously used services and receive the remainder value on your Luminate account to be used on a future service.
